Not sure what the context of this photo is but the depth of field is quite small leaving a lot blurred out aside from the back of the hand. I could see this having a much greater visual effect if the arm and fingers were more in focus and the background was something like sand or water surface as if it were reaching out for a last cry of help. Leaving it monochrome is fine too but I think it just needs more focus so you get more of the arm and hand and a fitting background to go with it. Something that also might be interesting is having another person in the picture thats being reached towards but is blurry as well, the context of that could vary so there's a lot of room to make different messages with this picture.
